Rahill Dental is required by law to maintain the privacy of protected health information, provide notice of our legal duties and privacy practices, and notify affected individuals following a breach of unsecured protected health information.

This Notice of Privacy Practices is effective as of April 29, 2026, and will remain in effect until it is replaced. Rahill Dental may update this notice when permitted by law. If a significant change is made, the updated notice will be posted clearly at our practice location and made available upon request.

How We May Use and Disclose Health Information

We may use and disclose your health information for treatment, payment, and healthcare operations. These uses allow us to provide care, coordinate services, obtain payment, and operate the practice responsibly.

Treatment

We may use and disclose your health information to provide, coordinate, or manage your dental care. For example, we may share information with a specialist, laboratory, pharmacy, or other provider involved in your treatment.

Payment

We may use and disclose your health information to bill and collect payment for services we provide. For example, we may send information to your dental insurance plan to obtain benefits, coverage information, or payment.

Healthcare Operations

We may use and disclose your health information for activities necessary to operate our practice, such as quality improvement, staff training, licensing, compliance, business management, and patient service activities.

Other Permitted Uses and Disclosures

We may also use or disclose protected health information when permitted or required by law, including for public health activities, health oversight activities, legal proceedings, law enforcement purposes, workers' compensation, disaster relief, research approved under applicable rules, organ and tissue donation, coroners or medical examiners, and specialized government functions.

We may disclose relevant information to a family member, friend, caregiver, or personal representative involved in your care or payment for care when allowed by law and appropriate under the circumstances.

Certain information, such as genetic information, mental health records, substance use disorder records, or other sensitive information, may receive additional protections under state or federal law. We will follow applicable special protections when they apply.

Text Messaging and Electronic Communications

If you choose to receive text messages from Rahill Dental, we may use your phone number and communication preferences to send appointment, scheduling, care-related, or practice-related messages.

We do not sell SMS opt-in data or consent information. We do not share text messaging originator opt-in data or consent with third parties for unrelated marketing purposes.

We may share information with vendors that help us provide messaging services, such as messaging platforms, phone companies, and service providers that support delivery of text messages, only as needed to provide those services.

Uses Requiring Written Authorization

Except as otherwise permitted or required by law, we will obtain your written authorization before using or disclosing your protected health information for purposes not described in this notice.

Your written authorization is generally required for the use or disclosure of psychotherapy notes, for marketing communications when required by law, and for the sale of protected health information.

You may revoke an authorization in writing at any time. If you revoke an authorization, we will stop using or disclosing your information for that purpose, except to the extent we have already relied on the authorization.

• Access: You may request to inspect or obtain a copy of your health information, subject to limited exceptions.
• Amendment: You may request that we amend health information you believe is incorrect or incomplete.
• Accounting of disclosures: You may request a list of certain disclosures of your health information.
• Restrictions: You may request additional limits on certain uses or disclosures of your health information.
• Confidential communications: You may request that we communicate with you by alternative means or at an alternative location.
• Breach notification: You have the right to be notified following a breach of unsecured protected health information as required by law.
• Paper copy: You may request a paper copy of this notice, even if you have received it electronically.

Requests, Complaints, and Contact

Requests to access, amend, restrict, or receive an accounting of disclosures of your health information should be submitted in writing to Rahill Dental using the contact information on this page.

If you believe your privacy rights have been violated, you may file a complaint with Rahill Dental or with the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
